To reduce the paperwork requirements when you arrive for your first visit at Team K9 Fitness, LLC, you can complete the following forms either on your computer and then print them, or print them and handwrite in the requested information.

Terms and Conditions
All clients of Team K9 Fitness, LLC must read and sign the Terms and Conditions form.

Registration Form
All dogs must have a registration form on file. If you have multiple dogs you will be bringing to Team K9 Fitness, LLC, simply complete the registration form and the Multiple Dog Form for each additional dog.

Multiple Dog Form (Registration Form Must be completed as well).
After completing the Registration Form, you can complete the Multiple Dog Form for each additional dog. The multiple dog form simply asks for only the dog's information for each additional dog, saving you time from writing your contact information and your veterinarian's contact information on each form.
